Insurance Information

If your insurance plan is listed below, Columbia University Pathologists will submit insurance claims for all pathology services. You are responsible for all co–payments, deductibles, and non–covered services as indicated by your insurance plan.

Payment

If you receive payment for the biopsy charges from your insurance carrier, please endorse and forward the check to Columbia University Pathologists along with your insurance carrier’s Explanation of Benefits (EOB).
